Human kidney failure

Patient’s with Chronic Kidney Disease or on dialysis are prone to anemia, infection and frequent hospitalization if not managed well.

Our holistic approach to disease process includes a Clinical Partnership with the in-center facility or the Renal Physician. This can be achieved through our interdisciplinary team—Nursing, Physical Therapy, Occupational Therapy, Speech Language Pathologist, Medical Social Work, our Consulting Renal Dietitian and our consulting Renal Social Worker.

The goals of treatment are:

  • Improve or maintain hemoglobin count
  • Improve or maintain diet compliance
  • Decrease incidence of fluid overload
  • Improve or maintain independence with ADL’s and IADL’s
  • Improve overall muscular strength and physical endurance
  • Improve overall mental outlook with the disease
  • Assess DME needs at home
  • Address any current symptoms (pain, weakness..)
  • Keep patient integrated with community activities
